Wrong affiliate getting credit

I'm a newbie. I just installed PAP and have entered a Campaign, Banner and Affiliate. When I make a purchase through the new affiliates banner, the new affiliate receives an email notifying of the new purchase, but the credit in PAP>Transactions goes to the affiliate that was pre-installed when I installed PAP. I've read the manual and watched videos and obviously think I've done things correctly. There are many items in Debug history I don't understand; one that stands out though is:
Registrator: Data/Cookie is empty
Is this not working because I'm testing from my computer or what am I doing wrong?

This problem is in the click tracking code. It has to be placed into each page, which will be destination page of your banners/links, If this code is missed, sales will be tracked, but commission will be assigned to the incorrect affiliate. This code you can find in tools > settings > integration wizard > step 2

So we have to place this in ALL of our web site pages or just the one page that the banner lands on when clicked? Because I am experiencing the same problem sometimes.

If your banner destination page is: www.your_site.com/page1.html and www.your_site.com/page2.html, then both pages has to contain this code
